Unfortunately, most morbidly obese people eat due to psychological reasons. It's an addiction, and when you are treating somebody who is in this situation, they have to be mentally prepared as well as physically. I weight the same as I did in high school and I'm almost 50 now. It gets harder as we get older, your metabolism slows down. The key is exercise. You have to get out there and move. The "some people have a gland problem" argument is actually augmenting the problem. At the very least it is misapplied and at the most it is out right inaccurate. The human body was designed evolutionarily to survive on far fewer calories than Americans take in. One small candy bar can lift a 230 lb. object for over a half a mile. That is how efficiently we use food. But folks are never gonna decrease their calorie intake. It's just not gonna happen.

I don't really get what that has to do with his problem. Even if that is the way he got to that weight, it doesn't matter now. To other people, eat less and move more will only help people who are chubby. Anyone above that cannot move as much as average size people can, and are accustomed to the amount they eat. A lot of it has to do with socio-economic conditions as well. Crap food usually is a lot cheaper than healthy organic type stuff, so poorer people ironically can be fatter. I hold a certificate in nutrition, and yes it is possible that he was malnourished. I can only imagine the EXTREME amount of junk food and empty calories he must have been taking in! To weigh that much and not even get nearly the amount of nutrients needed for sustanance of the body; and little or no exercise, I'm sure. In my opinion the flaw in this video is the critical time since the insertion of the ET tube and the ventilation, the practitioner hesitate in that process first wanted to connect the oxygen to the tube... hesitate and then had to disentangle the tubes of respirator! everything "must" be prepared for that procedure... and more if you are filming it, its my point of view. but is the reality when you are in that situation, in general its a good procedure.

Do you know the name of this device, or who manufacters it? It looks like it is a breathing spontaneously technique. Is this the standard technique for a difficult airway? If so, which organisation has published the algorythm. Is fibreoptic intubation no longer used? Sorry for so many questions. I am an anaesthetist in the UK and this is not a standard technique that we use here.
